    Promoting dignified end-of-life care in the emergency department: a qualitative study

    Background: Preservation of a dying person’s dignity in the emergency department (ED) is fundamental for the patient, his/her relatives and healthcare professionals. The aim of this study was to explore and interpret physicians’ and nurses’ experiences regarding conservation of dignity in end-of-life care in dying patients in the ED. Methods: A qualitative study based on the hermeneutic phenomenological approach, was carried out in the emergency department of two general hospitals. A total of 16 nurses and 10 physicians participated in the study. Data collection included 12 individual in-depth interviews and 2 focus groups. Results: The findings revealed that two themes represent the practices and proposals for the conservation of dignity in the emergency department: dignified care in hostile surroundings and the design of a system focused on the person’s dignity. Conclusion: Dignifying treatment, redesigning environmental conditions, and reorienting the healthcare system can contribute to maintaining dignity in end-of-life care in the ED

    Annual Legumes as an Alternative for Animal Feeding in Cuba

    Studies conducted in Cuba have demonstrated the importance of the agronomic and nutritional performance of the species Vigna unguiculata (cowpea), Canavalia ensiformis (jackbean), Stizolobium niveum (mucuna), Lablab purpureus (dolicho) and Glycine max (soybean) as feed sources for non-ruminant species. Under Cuban tropical conditions, and with minimum agricultural inputs, jackbean, dolicho and mucuna have attained forage yields between 4 and 6 t dry matter (DM)/ha and grain yields between 2.57 and 3.41 t/ha and cowpea and soybean have given yields of between 1 and 2 t/ha (DĂ­az 2000).     Aims: to explore and understand the experiences of terminally-ill patients and their relatives regarding dignity during end-of-life care in the emergency department. Background: the respect given to the concept of dignity is significantly modifying the clinical relationship and the care framework involving the end-of-life patient in palliative care units, critical care units, hospices and their own homes. This situation is applicable to in-hospital emergency departments, where there is a lack of research which takes the experiences of end-of-life patients and their relatives into account. Design: a phenomenological qualitative study. Methods: the protocol was approved in December 2016 and will be carried out from December 2016 to December 2020. The Gadamer's philosophical underpinnings will be used in the design and development of the study. The data collection will include participant observation techniques in the Emergency Department, in-depth interviews with terminally-ill patients and focus groups with their relatives. For the data analysis, the field notes and verbatim transcriptions will be read and codified using ATLAS.ti software to search for emerging themes. Discussion: emerging themes that contribute to comprehending the phenomenon of dignity in end-of-life care in the Emergency Department are expected to be found. This study's results could have important implications in the implementation of new interventions in Emergency Departments. These interventions would be focused on improving: the social acceptance of death, environmental conditions, promotion of autonomy and accompaniment, and assumption (takeover) of dignified actions and attitudes (respect for human rights)

    Metastable liquid lamellar structures in binary and ternary mixtures of Lennard-Jones fluids

    We have carried out extensive equilibrium molecular dynamics (MD) simulations to investigate the Liquid-Vapor coexistence in partially miscible binary and ternary mixtures of Lennard-Jones (LJ) fluids. We have studied in detail the time evolution of the density profiles and the interfacial properties in a temperature region of the phase diagram where the condensed phase is demixed. The composition of the mixtures are fixed, 50% for the binary mixture and 33.33% for the ternary mixture. The results of the simulations clearly indicate that in the range of temperatures 78<T<102o78 < T < 102 ^{\rm o}K, --in the scale of argon-- the system evolves towards a metastable alternated liquid-liquid lamellar state in coexistence with its vapor phase. These states can be achieved if the initial configuration is fully disordered, that is, when the particles of the fluids are randomly placed on the sites of an FCC crystal or the system is completely mixed. As temperature decreases these states become very well defined and more stables in time. We find that below 90o90 ^{\rm o}K, the alternated liquid-liquid lamellar state remains alive for 80 ns, in the scale of argon, the longest simulation we have carried out. Nonetheless, we believe that in this temperature region these states will be alive for even much longer times.Comment: 18 Latex-RevTex pages including 12 encapsulated postscript figures.     Sub-barrier fusion of 6He with 206Pb

    Cross-sections for the production of 210Po nuclei in 6He + 206Pb collisions over the incident energy range 14–18MeV were measured by means of the activation technique and a radiochemical analysis. The elastic scattering at 18.0MeV was also measured providing a precise value for the 210Po production cross-section at this energy. The results are at variance with the earlier experimental data and rather in accord with the predictions of a density-dependent barrier penetration model for the fusion process. A proper treatment of beam energy distribution for the evaluation of the activation data is discussed

    Soft X-ray emission lines in the X-ray binary Swift J1858.6-0814 observed with XMM-Newton-RGS: disc atmosphere or wind?

    We find soft X-ray emission lines from the X-ray binary Swift J1858.6-0814 in data from XMM-Newton-RGS: N VII, O VII and O VIII, as well as notable residuals short of a detection at Ne IX and other higher ionisation transitions. These could be associated with the disc atmosphere, as in accretion disc corona sources, or with a wind, as has been detected in Swift J1858.6-0814 in emission lines at optical wavelengths. Indeed, the N VII line is redshifted, consistent with being the emitting component of a P-Cygni profile. We find that the emitting plasma has an ionisation parameter log⁥(Ο)=1.35±0.2\log(\xi)=1.35\pm0.2 and a density n>1.5×1011n>1.5\times10^{11} cm−3^{-3}. From this, we infer that the emitting plasma must be within 101310^{13} cm of the ionising source, ∌5×107rg\sim5\times10^{7}r_{\rm g} for a 1.4M⊙1.4M_{\odot} neutron star, and from the line width that it is at least 104rg10^4r_{\rm g} away (2×109(M/1.4M⊙)2\times10^{9}(M/1.4M_{\odot}) cm).     GW150914: First search for the electromagnetic counterpart of a gravitational-wave event by the TOROS collaboration

    We present the results of the optical follow-up conducted by the TOROS collaboration of the first gravitational-wave event GW150914. We conducted unfiltered CCD observations (0.35-1 micron) with the 1.5-m telescope at Bosque Alegre starting ~2.5 days after the alarm. Given our limited field of view (~100 square arcmin), we targeted 14 nearby galaxies that were observable from the site and were located within the area of higher localization probability. We analyzed the observations using two independent implementations of difference-imaging algorithms, followed by a Random-Forest-based algorithm to discriminate between real and bogus transients. We did not find any bona fide transient event in the surveyed area down to a 5-sigma limiting magnitude of r=21.7 mag (AB). Our result is consistent with the LIGO detection of a binary black hole merger, for which no electromagnetic counterparts are expected, and with the expected rates of other astrophysical transients.Comment: ApJ Letters, in pres

    Dips and eclipses in the X-ray binary Swift J1858.6-0814 observed with NICER

From these, we find an orbital period of P=76841.3−1.4+1.3P=76841.3_{-1.4}^{+1.3} s (≈21.3\approx21.3 hours) and an eclipse duration of tec=4098−18+17t_{\rm ec}=4098_{-18}^{+17} s (≈1.14\approx1.14 hours). We also find several absorption dips during the pre-eclipse phase. From the eclipse duration to orbital period ratio, the inclination of the binary orbit is constrained to i>70∘i>70^\circ. The most likely range for the companion mass suggests that the inclination is likely to be closer to this value than 90∘90^\circ. The eclipses are also consistent with earlier data, in which strong variability ('flares') and the long orbital period prevent clear detection of the period or eclipses. We also find that the bright flares occurred preferentially in the post-eclipse phase of the orbit, likely due to increased thickness at the disc-accretion stream interface preventing flares being visible during the pre-eclipse phase. This supports the notion that variable obscuration is responsible for the unusually strong variability in Swift J1858.6-0814.Peer reviewe
